A Clinical Study of the Acute Appendicitis


Abstract
The collected data of 997 patients who were operated on under the diagnosis of acute appendi-citis at the Seoul National University Hospital during the last 6 years and 6 months (1970.1-1976.6) were studied to review some clinical aspects of acute appendicitis.
Following results were observed.
1. In 859 out of total 997 cases, the preoperative diagnosis of acute appendicitis were correct and among them 253 cases were complicated appendicitis. Thus the diagnostic accuracy. was 86.2% and the incidence of complicated appendicitis was 29.5% in our series.
2. The sex ratio (Male: Female) was 1.2: 1 and the incidence of acute appendicitis was most prevalent in the 2nd (28.4%) and 3rd (25.8%) decades. But the relative incidence of complicated appendicitis was most high in the 5th decade (48.0%) followed by 6th and over the age of 61.
3. While 72% of the patients with simple appendicitis were admitted to this hospital on the first or second day of the onset¢¥ of symptoms, most of ¢¥the patients (72, 5%) with complicated appendicitis were, admitted on the third day or more of the onset of symptoms.
4. In 75% of acute appendicitis the leukocyte count was over .10, 000/mm3, but there was no significant difference in the degree of leukocytosis between simple appendicitis and complicated appendicitis.
5. One, patient was died of acute appendicitis complicated with diffuse peritonitis. (Operative mortality; 0.12%) Wound infection rate was 5.3% in simple appendicitis and 32.3% in compli-cated-appendicitis.
6. The mean number of hospital days were ¢¥6.6 in simple appendicitis and 13.1 in complicated appendicitis.
